WOW FACTOR BASEBALL PARTICIPATION RELEASE & WAIVER OF LIABILITY To be completed and signed by the parent or legal guardian of the participating minor athlete prior to tryout participation.

ACKNOWLEDGMENT & ASSUMPTION OF RISK

As the parent or legal guardian I, acknowledge that participation in baseball tryouts, training, and related activities involves inherent and other risks of physical injury, including but not limited to risks from running, sliding, throwing, batting, fielding, contact with other participants, and being struck by thrown or batted balls. I understand that these risks cannot be eliminated, regardless of the care taken to avoid injury. I as the guardian voluntarily allow the minor to participate with full knowledge of these risks and assume all such risks on behalf of myself and the minor.

RELEASE & WAIVER OF LIABILITY

In consideration of the minor being permitted to participate, I hereby release, waive, and discharge Wow Factor Baseball, its owners, directors, coaches, staff, volunteers, sponsors, and affiliated organizations (the "Released Parties") from any claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by the minor or by me while participating in the event, to the fullest extent permitted by applicable law. I further agree to indemnify and hold harmless the Released Parties from any claims brought by or on behalf of the minor as a result of the minor's participation.

In the event of injury or illness and if I cannot be reached, I authorize the Released Parties to obtain or administer emergency medical care deemed necessary for the minor, and I accept responsibility for any costs of such care.
